Intimate the ESIC officials in Form - 16 immediately, on receiving form 16 they will ask for the following documents :
1. proof of employment of employee
2. wage register
3. attendance register
4. witness report
if required they will seek explanation letter from Employer. further Form 32 is to be submitted on demand.

In case of Minor accident, You have to send accident report to ESI within 24 hours to Local office of ESI.
In case of Fatal/Major accident, you should summit accident report through a special messenger,
After of receipt of accident report, the ESI authorities will make enquiry and they will submit report to ESI for further activities.
