As per The Contract Labour (Regulation & Abolition) Central Rules 1971, Form X is "Application for Temporary Licence" and not Identity Card. If you want to have the format of Identity Card, in this site itself it is available. Please do search and you can get it.

If your company comes under the SEZ area, then you must issue Identity Card to all the workers engaged by you as per Form K of SEZ Rules, which I have reproduced in this post.
