The design depends upon your objectives. May I know why you are asking demographic questions. If you have to seek personal info, it's better to do it in the end. You also should tell the respondents why you are seeking their help; it will also help to improve the response rate if you can tell them how they benefit from investing their time in answering.
I would like you to put your thinking cap and ask questions about why you are using different scales and whether your scales are balanced or skewed. Please also check for changes in the tense of the words used in your scales.

i think demographic questions are asked to know that whether the person has the knowledge or is apt to be considered for collecting data...i will shift them at the end..what do you mean by scales?n their being balanced or skewed?

Please compare the scales used for Q7 and Q11. Q11 scale is balanced as it has ratings for agree and 2 for disagree, Where as Q7 has 3 for agree and 1 disagree. I termed the former as 'Balanced' and the latter as 'Skewed'.
