Dear Baneet,
Form 12A under Employees Provident Fund & Misc. Act is the monthly return of remittance.
It is submitted on monthly basis along with Form 5 ( New Joinee Detail) and Form 10 (Leving employees details) , Form -2 (Nomination & Declaration) , Triplicate copy of challan.
Monthly Return of E.P.F. normally contains the follwing
1. Form -2 (for New member during the month)
2. Form - 5 (detail of new joinee during the month)
3. Form -10 (detail of left employees during the month)
4. Form -12 A( detail of remittance as per challan)
5. Triplicate copy of challan (thru which monthly contribution has already been deposited on or before 15th of the following month).
