Dear Anupama
Do you have that much Disciplinary Cases pending in your company, which made you to think of forming a disciplinary committee?
Please make a Separate Chapter in your HR Manual for Discipline
In that you make what are the cases are considered as Disciplinary cases those attracts disciplinary action or administrative action.
What are the Punishments you can award, (Minor Punishment / Major Punishment)
Nominate some Presiding Officer and 2 members.
Presiding Officer must be GM level or the person authorised to conduct and Member must be 1 his superior Officer and 2 is HR Person.
You can conduct as Board of Inquest and as deemed fit you can award punishment which will be taken into consideration during their appraisal time.
If you think you will have lot of ideas conceived in your mind.
Think and prepare yours and get the same approved from your appropriate authority and add in your HR Manual (as like addendum or Annexure to HR Manual or Chapter in HR manual).
Wish you all the best for your innovative idea
With warm regards

Dear Anupama
If the female employee is undergoing some disciplinary proceedings then the either member must be a female employee (Either HR or Section Head), who can understand and explain the problems and grievance of other female employee.
That's it.
