Hi Nazar,
Ideally this is not something you should be doing. You should let management arrive at a decision first. Also, you should first complete the three years and only then, if you don't get an increment, should you intimate management about your not having gotten an increment.
As regards sending a letter, there is no set format for such an increment letter. It is up to your discretion and based on your management and type of company, that you should write the letter.

Hi Nazar,
I think you want to write to your management to consider an increment upon completion of your 3 years service.
In case your organization does not have a system of Appraisals, Increments, Annual Performance Review, etc.... then you can write to your management to increase your salary.
Your application should contain much about your performance during the last 3 years, specific achievements, contribution to the organization, the responsibilities that you are handling...... so on.
Hope, this is in line with your requirement.

Hi!
my superior asked me to write a letter to him so that he can help me to get increment as he wants me to stay with his team, below is my draft and need your help to improve it ......thanks....
Dear Mr XX,
I have been working with XX for almost 1 year and my contract with XX is going to due on xxxx.
Now I am getting XXX as my monthly basic salary. I am confident that I have put in my best effort during the last few months in contributing to the project team and my skills are improved during this period. I would like to be considered for an increase in salary at this time.
Below are my achievement & contribution to the company during past few months:
- Liaise with sub-contractors & project team in documentation & site folders
- Checking & compiling over 200 site folders before submission to client
- Filling & keeping softcopy data in company server for over 200 sites
- Coordinate the ordering of office supplies & hand phone bills
- Preparing delivery orders, transmittal letters & office administrative work
I~{!/~}m not afraid to assume responsibilities. I also working with minimum supervision meet tight deadline. Hopefully you can provide your recommendation to XXXX from HR department for their further action.
Hope this application will merit your kind and favorable consideration.

The sample format which Kumar has given is a very good one based on that nazar have to develop your own increment letter.
Normally if anyone is going to request their increment it should portray their importance in the organisation and what they have contributed in the past present and what they are going to contribute in the future..
It should not be a formal increment letter as it looks odd..
Also I have given letters for increment for employees (as if they are writing it )as they were not aware how to do it but once it is approved by the Management then only I give them the formal increment letters.

If you think you are Eligible to get higher Salary and your company / Management are not in condition to Increase Due to recission... Its better to search for New Job and one more thing Change Management will always help candidate to Increase his / her Salary....
