Hi,
I'm working in a publishing company as an HR Executive. We have just finished with our yearly appraisals in the last month. The employees of my company have been given certain objectives to be achieved over a period of time (many have monthly objectives to be achieved). I have been asked to design a post-project appraisal form wherein a track will be kept if the objectives are being achieved or not, so it is easier to take the required action in case they are not being met.
Can someone plz help me with designing the form. Plz answer ASAP since this is one of my objective :icon1:, & I have never designed any such form earlier.

I am a retired academic and I also only search the web for info. Please send me details of the system followed by you, I can educate myself.
