Hi guys... In my office one employee hospitalised by road accident when he was going to home. I want to know whats the ESIC process of Accident. I given the form no 12, now he called me and said madam give me the MER card. And one thing what to do for the his leave compensation? Guys please help me I'm newly join this filed so plz reply me as soon as possible.

@Amrita-Sawairam, I don't know what is MER card but what you have to do in the case of accidents while on duty and during the course of employment including road accidents while commuting to office or back to residence if the notional extension theory is applied with respect to time and place of accident is that you have to submit form 12 (that you have already submitted) and to submit a FIR and a sketch of the place where the accident has happened. This is just an ordinary sketch showing the roads from the plant/ office, leading to the residence, the landmark where the accident happened. You should also submit a statement by a witness as to what has happened. Normally, in the case of injury within the plant some body in the company will be giving that statement but since it is a road accident there may not be any eye witness from the company but there can be someone who has seen it. This is apart from what is mentioned in the seen mahsar and FIR.

But the basic thing is done, ie, submitting form 12 online. Now, you can get the above mentioned things and submit them offline. If the injured is admitted in private hospital, ask any of the family members to go to the Dispensary and inform the accident, preferably collecting the dispensary copy from you. The print out of Form 12 is usually sent to the Local Office and the Medical Officer also.
