here i m attaching a sample of daily shift report formate that we use in our company which is filled shift wise & a combined report is circulated in the morning to all for corrective & necessary action.
you can modfy it as per your plant requirment.

we have citation book in 3 copies whose contents are below:
company's logo & name
Unit:
Date & time:
Name of person:
Gate pass no./Emp. No.:
Sub contractor:
Nature of safety violation:
Plant/Location:
action Taken:
Remarks:
Signature
Name of safety officer
this formate id filled in three copies, one is given to the person who is imposed penality,one slip is send to finance deptt.who deduct the money from there salaries or contractors work order,and one is kept in safety office for record.
monthly basis we compile data & send slips to finance & there money is deducted.
a complilation sample report is attached.

Dear abhay,
it depends on nature of voilation.if it is not serious than we give oral warning else we directly deduct it. For example, if helmet is lying near him & he is not wearing than we warn orallyif he wears than ok,again if we visit & he is not obeying than written warning.
If he do not at all hv helmet at the site than penallise.
