Hence, here is some advice. You need to clearly state what is the benefit for the people who take the time to answer your questions. For example, you could say that you will publish the conclusions of your study at Citehr.
I tried to answer your questionnaire and found that the options given are not consistent. For some questions, you have give an option "other". May be, in trying to keep the questionnaire short, you have listed a number of characteristics for some questions. That made it difficult for me to choose an option, when only three were given and the option "Other" was not included.
