Need to prepare the letter format which should include promotion cum increment cum confirmation details in the same letter only

Hi, While increment cum promotion letter is very common adding confirmation also in the same letter looks abnormal. How long is your probationary period ?

Probation period here is 6 months
The company is startup, as here the HR was not appointed any time before hence the employee did not received the confirmation letter rather any official letter before this.
Now when I have been appointed here , employee are asking for same.
So need such format where all things can be mentioned.
