form 12A is a register or as per my knowledge is what we cal a "muster roll" in which all the details regarding the employee and the employer contribution in the PF account of an individual is mentioned..:!:

The form 12-A is a monthly return of EPF act,1952. It's compulsary for employer to submit form 12-A in PF office before 21 of every month.

As per my understanding, you are talking about the 12A from Provident Fund, right..
Form -12A is a consolidate statement of dues & remittance of all the employee whether Joinee or Resignee and it has to be submitted to Regional P.F office along with Form - 5&10, within 20 days of the close of the month..
