This might be a long query but I would appreciate if you have the time to spare and go through it.
I joined Infosys on 26th October, 2015 as a System Engineer trainee. I left Infosys on 10th November,2015. The reason was a serious medical condition of my mother. I came back home and also informed the Infosys HRD of the same. They immediately granted me leave. I also informed them of the fact that considering the seriousness of the issue, I might have to resign as well. Now after coming back home, I continued to upload my mother's medical documents to Infosys at regular intervals, thereby extending leaves day by day. Not the latest reports did not seem very well. The Doctor was unable to provide me with any good news. I was really heartbroken and my health is declining as well due to anxiety and sleepless nights. I opted for resignation from Infosys on around 27th Jan,2016. Infosys immediately complied and gave me the separation formalities and all. In the meantime, I thought that I have to ope for a job in my home town. Even a small company would do considering my serious situation at the moment. My father's retired. Mom's medical bills are huge. I have to opt for a job. Now I actually landed one. Not a very big company though but something's better than nothing, isn't it? This new company is asking me to fill up their EPF form. Now, I have not shown them my previous company experience of 14 days ! I reckoned that 14 days' of experience counts nothing. Plus Infosys is a MNC...so I thought that maybe I could be rejected by my new company on the basis of past experience with a such MNC, such a reputed company ! Anyway, I applied to them as a Fresher and got selected. Now, in the new company's EPF form I can see something like this - "Did you word in any EPF related organization? If yes write the UAN number or PF account no. etc etc". Now I don't want to provide my previous employment details. Plus Infosys takes around 45 days to send me the relieving letter and I have joined already. So around next month I will have the offer letter, I guess. What should I do? Should I lie on the new company's form marking the option"No"? I am actually the worst liar in the World. I know that it's unethical but consider my situation Sir/Mam. I was in turmoil.
Would be glad to have your suggestions.

P.S. I had not signed any EPF related form with Infosys during my 14 days in thier company. I don't know my PF account number with them as I have not received my 14 days' pay-slip. The salary was credited though. I don't know whether they deducted my 14 days' PF amount or not. Now, can Infosys already make a PF account in my name without my signature on any form of theirs? And will that have a UAN?
I thought that maybe they just deducted the PF amount from my salary hoping to deposit it in my previous account after I come back and sign their PF forms. This is my only guess. In my separation formalities with Infosys, there was a PF form which is a form of withdrawal of "EPF" money. I don't want my money. I don't want to transfer it as well. Just let it be. Infosys has a PF trust. My new company is registered directly to EPFO.
How can any company make a PF account for his employee without his/her signature? Please help me out.

It pains me to leave Infosys to be honest. I was in love with the company when I first placed my foot in the Mysore campus. Seemed like a heaven to me. I thought that my career is sorted for the future. But sadly, God thought otherwise !

For enrollment EPF enrollment, it is responsiblity on part of employer to deduct pf and enrolling for Epf. Without your signature, you can b allttd the number. You also can't deny for Epf cobtribution except
if u have submtd form 11 that, earlier Pf was not ddctd en now u R drawing basic salary more than 15k.
You have to approach company(employer) for submission of form 11. If Infosys openedvur acnt with EPF, is not rejectable.

What is process of UAN in case of a ERP trust? Do they get UAN number?
Plus, I have not actually signed Infosys EPF form regarding my account. So can I say to my new company that I have not worked in any company with EPF contribution?
