My suggestion is to prepare a format with the following details:
1. Sl.No
2. Name / group (who would require training)
3. Designation
4. Topic
5. Preferred time & duration
6. Preferred Trainer
This form could be send to all dept heads, so that they can fill it up and send back to Training dept. Once all the forms are received, the calendar could be prepared. It may not be practical to design one questionnaire that would do justice to the requirements of all depts
