dear Vichitra,
yes, you can but you must also know the following things:
1.you must wait for atleast two months for transfer of funds.
2. You must know that if you didnot fill the form correctly or you miss something then form will be return by PF office to the address you gave or to the organization you were working.
if you can beleive to HR deptt of your previous co. then stay a touch with them, otherwise send your form of your own.
