Dear All, It is very necessary to have a perception survey of all employees regarding safety at regular intervals. The form is attached herewith.
Do it at your end so that you can have a better judgement regarding the Safety Culture / Safety Practices / Safety Improvements needed etc.
I have done in the beginning of the Plant Start-up (before 8 Months) and Last week again I have done. The result shows the difference between the employees' understanding related to safety.
You have to do a safety survey for all employees including Contract employees in the local language also.
Give your comments. You can add more points / change any points.

Thanks for initiating such a thread and for sharing good survey check list... This kind of survey was done in our plant by M/s. DuPont as an initial activity towards the Safety Excellance Journey... It was called Safety Perception Survey...
Almost all the points you are covered... You may add upon the below points:
1. How offen structured training programes are conducting in your organization?
2. Whether you find these training be of any value?
3. How do you rate the working of your safety dept?
4. Do you beleive that the accidents are causing as the Safety Dept. is not effective? Etc...
If you really wana do any further modification do write to me back... I can share with you a good document on this...

Very good information. We previously conducted an evaluation during "Behaviour Based Safety" training, covering some of these questions. But fully not covered. After viewing this information, am planning to organize the same in our organization.
I am sure that this type of Survey may realise many Unsafe Behaviours.
