Hi, I'm on a sabbatical due to health issues and since my company is not willing to extend my leave for a couple more weeks I've decided to resign from my position. But on discussion with the HR, they've stated that they can only terminate me and not let me resign since now I won't be able to serve my 90 days notice.
What will this do to my career profile down the line if they'd terminate me from my current position? Any advice on how to tackle this would be helpful.

Better negotiate with HR for proper relieving instead of termination. You can check regarding pay in lieu of notice option. Future Employers might not prefer candidate with termination background though you have valid reason of ill health. Even this they might perceive negatively (about your physical fitness to execute the work)

Firstly, about removing the misconception about "sabbatical". The meaning of a sabbatical is: Sabbatical leave is a period in which a person does not report to their job but is still employed by their company. Sabbatical leave is usually taken by employees who want to pursue personal interests, such as studying, travelling, writing, and volunteering. Sabbaticals can be paid or unpaid but are essentially just extended periods of leave.

If you are unable to work due to health issues, then it is "Medical Leave" and not "Sabbatical" as such. The medical leave could be paid or unpaid.

Basically, your question is about the impact of the termination of employment on your future career. I recommend you approach MD to condone fulfilment of the condition of serving a 90-day notice period. Your earnest plea should influence him/her so that he waives off this condition. Explain to him/her why you cannot attend to the duties and how the termination from employment could deprive you of future job opportunities.
