One of our employees worked as Administration Assistant cum receptionist previously before she requested to be promoted to Acting HR & Administration Executive with a salary increase. However, she is unable to perform during the duration of her role as HR & Admin Executive. As a result, management had reinstate her position back to Administration Assistant cum receptionist but without changing her salary. Her increment still remain the same.

Here, I would like to know will there be a sample of letter that I can write to this employee that her position is reinstate to Admin Assistant cum receptionist with salary remaining unchanged?

its very difficult. even for demotion you can only demote to the lower position but the salary must not reduce.
by rite you should issue a promotion letter with 3 months probation period. open confirmation after 3 months probation then you may give her salary increment.
in your case i suggest remain the salary at the moment. in the event your company give annual increment or annual bonus you may exclude her for the above reason.
