The License is mandatory in your case as you fall under " Factory Definition" under the Provisions of the Factories Act 1948.
1. You need to look for the relevant Form under Factories Rules applicable for your State and use the format by adding annexure to the form as required and attaching manufacturing process flow chart, list of board of directors as well as by remitting the Fee. The Fee is mostly based on the horse Power used and number of Manpower used.
Most of the State Governments now made it simple and on-line. You may visit the website of the Department of Directorate of Industrial Safety and Health for more information and upload your application.
2. You need to get approval for the Building as well as Machinery Lay out under the Provisions of the Factories Act which needs to be done by submitting the blue prints either on-line or off line. You may consult your location Asst Director of Industrial Safety and Health
3.You once license is issued you need to intimate the name of Manager after appointing any internal Manager of your Factory as designated " Factory Manager " under Factories Act 1948.
