I do not have the flow chart.
Pl look at the following thread.
I have attached a rule and regulation manual where the permit conditions are mentioned.
you can prepare flow chart yourself.
If you require any help in preparation of the same pl let us know.
Once you make it request to upload for the benefit of all.

Flow chart is like this :
Originator (Plant Supervisor, plant in charge)- safety incharge- HOD of the originator- originator (initiator of work permit)- receiver (Executor of the job or the contractor to whom the work is entrusted).
Once work is over, it has to be returned and closed with safety incharge signature.
There are two types of work permit. (Class 1 which involves hot work and vessel entry, working in heights, and Class 2 which is not as intensive as Class 1 Jobs.
Hope the above is in line with your requirements.

Dear Murdhar,
Training Department has to give course to the issuers and receivers for work permit. After course we have to do practicals and then certify them. After 2 years again recertifiy them.
