Warning letter to employee for giving wrong information

Hi,

Who issued wrong discharge summary to the Patient. Either Nurse from the Ward or staff from Admin section?

Accurate information is needed for such warning letters. Please refer the following template. You may edit as per your requirement.


Dear _______

Sub: Warning for negligence of work

It has been brought to our notice that on _____(date) you have issued a wrong discharge summary to our Patient Mr/Mrs._______ (due to your negligence) who got discharged from our Hospital on_______ . This error had taken place primarily due to your carelessness which could have been avoided if you had reviewed the file before issuing it to the Patient. This is not expected from you and not at all acceptable.


The above Act of yours had tampered the fame of the Hospital and brought disrepute to the whole Team. We hope you will understand the seriousness of this issue and you are hereby cautioned to take utmost care, responsibility and ownership of entire work process and ensure no stone unturned in future. You are hereby directed to take serious steps to improve your work process and have some checklist in place. In case of any further repetition you will be held liable and penalized for the errors and the Management will proceed with further disciplinary action.
