Dear Team, Employee continues chasing with ESI department they are not given proper information. Kindly help me on the below points

1) How do we claim ESIC maternity benefits from ESIC and which forms do we need to submit to the ESI department?
2) What are the documents we need to maintain form the employer?

An employee who needs to avail maternity benefits from the ESI should first ensure that she qualifies to get the benefits from the ESIC. She can very well get the same by visiting her IP profile. It is just 70 days' contribution over two consecutive contributions periods. If she satisfies this condition she will get the maternity benefits. She can first consult the medical officer of the dispensary chosen by her. The medical officer will examine the woman and issue the necessary reports which can be given to the employer. There are forms like form 19, 20 etc and the same will be provided to the insured from the dispensary itself. Therefore, what the employee is to do is to first visit the dispensary attached to her and consult he Medical officer. The rest, the date of expected delivery, day from which she should be on leave etc can be decided later on. The certificate issued by the Medical Officer will contain the date of expected delivery, and the same should be handed over the the employer.

Hi first of all you have to visit esic mapped dispensary after confirmation of pregnancy ,here dr.will check & provide certificate for the same.
Again when you have to start your maternity leave to have to collect leave certificate from same dispensary & after delivery you will visit dispensary again & your all certificates & form will be sent to esic branch office for claim settlement .
Branch manager will pay the compensation amount after the verification of all documents in your account.
