Can any one help me with the format of: Death Certificate while in service EPFO says that the above is to be issued by the principal employer.

The Death Certificate is normally issued by the Government authorities such as Municipal, Corporation etc., As an employer we are not supposed to certify a death of the employee as it would not bind legally. The other certificate such as Legal Heirship certificate can be obtained by applying to the Thasildar which are valid and as an employer legally it would not bind with the requirement.
