Can anybody help me to provide relieving letter format, there is one clause i.e. an employee worked with the organization for couple of years and then resigned and after few months again rejoined the company. In such case if we wants to add his working history in relieving letter, then what could be the letter format?

Dear member,
You say that the employee in question worked twice in your company. Therefore, he had two tenures in the company. Then at this stage, you are liable to issue relieving letter only for his second tenure. On completion of his first tenure, your company must have issued the relieving letter. Please check your office records.
Part of one of the sentence of your post is "if we wants to add his working history in relieving letter". Building working history of the employee is not employer's job. It is individual's. Therefore, do not be gratuitous while issuing the relieving letter.
