Are you talking about a leave form or a leave card? Be specific as there is a difference between the two. A leave form will only contain a leave request from the employee.
A leave card should have the complete record of the employee for the current year and a column stating the leave balance from the previous year. It should also include a column to calculate the leaves taken and the remaining balance at the end of the current year.
