You cannot rectify or change the name by yourself. You have to report to the local office of ESIC, and they will suggest or rectify it. Another option is that you have to take leave at the end of the month without attending.

No need to worry. Just add a new declaration form for the same employee. When filling in the contribution details, input zero wages for the previous ID and enter the correct wages under the correct ID. When stating the reason for zero wages, mention that the employee does not belong to the employer.
