Form 19 is for Provident Fund and Form 10C is for Pension. The above-mentioned forms are to be filled to get back your Provident Fund amount. Along with that form, you need to enclose your bank passbook/cheque leaf and any required ID proof.

Dear friend,

Nowadays, you need not submit the forms in the PF office. Your ex-company can submit them online. Please verify this with your nearby PF office. The online method is very easy.

Form 19 and Form 10C, along with a cancelled cheque if your name appears on the cheque or bank passbook, must be duly signed. You should also affix a Re 1 revenue stamp on both forms where required and sign across the stamp.
