Hi Manmitha,
Please check at the court campus. There is a standard and printed format available (If I am right, it's Form 19 A & C) that you have to fill and submit through your employer. After her comment, then only the PF commissioner will act on it. If the employer is not acting, then you can apply directly with a copy of proof to the PF commissioner.

There is no need to write a letter to PF authorities requesting PF forms. Claim forms (19 & 10C) may be downloaded from the website: www.epfindia.com and the same may be filled and sent to PF authorities. You are required to provide the following for the claim:
- S/B account number
- Revenue stamps 4 Nos (1 Rupee)
- Your PF number and branch
