You have to approach the Office of the P.F. Commissioner nearby to your company to obtain a set of forms for applying the EPF code number. Additionally, you need to submit copies of the attendance register, wages register, registration documents from small-scale industries, PAN card, lease cum rental agreement, and other related documents as specified in the forms.

Contractors should have a work order from the principal employer. The total strength should be more than 20 employees. Registration under the Contract Act with the Labor Department is required. The first invoice should be raised by the contractor. Registration certificates under various acts, i.e., labor registration under the Shop Act, VAT registration, PAN number, partnership/proprietorship particulars, and wage register under the Contract Act, should be provided by the contractor before the EPFO (Employees' Provident Fund Organisation), EO (Enforcement Officer), of the concerned area. An application to grant a code number under the EPF (Employees' Provident Fund) and MP (Miscellaneous Provisions) Act should accompany the submission.

Please go through the website of EPFO where the procedure/proforma for registration is available.
In short, the employer must have 20 or more employees and should not be attached to one single Principal Employer. There is a provision for voluntary coverage also in case the number of employees is less than 20, the proforma for which may also be seen on the EPFO website.
