As I am also on the same page, I wish to apply for PF change of address. They suggested to collect Form 5A available in the stockroom, fill it out, and then visit the Enforcement Office on Mysore Road. Submit a Covering letter and Form 5A, get the acknowledgment, and submit one more copy to your Regional office for another acknowledgment. They will update the address in their records.
