Actually, I need the TDS declaration form that we get filled from employees. In this form, employees fill in the amount of their investment plan in different sections for the following year: Mediclaim, PPF, Infrastructure, LIC premium, Children's tuition fee, Loan, Interest on loan, etc.
I have the form for the year 2011-12, but my colleagues have said that it is incorrect because a few sections have changed from this year, which I am not aware of. Changes have been made in Sec 80C, point 10, and infrastructure bond.
You can see the old one attached herewith.
Please help me.

I believe the Infrastructure Bond is now called the "Rajiv Gandhi Equity Saving Scheme," and the amount has been increased from 20000 to 50000 annually. This is the only change; all other slabs would remain the same.
Please confirm.
