The concerned employee has to submit an affidavit, get it published in a local newspaper, and finally, a Gazette notification is a must. He has to submit all the above three documents for his name change.

Nukani is right, you should ask for a gazetted copy. Only then will the employee receive their future benefits from the employer. If the employee has not applied for it, ask them to publish and obtain a copy.
