The example of a bolt falling on the head of a person without a helmet causing an accident is not exactly correct in my view. The incident initially is the falling bolt. If the bolt did not fall, there would be no accident. However, the impact of the bolt falling on a head without a helmet is greater compared to falling on a helmeted head.
So, all the helmet can do is reduce the gravity of the impact; it cannot prevent the bolt from falling. An unsafe act has caused the bolt to fall, leading to other consequences when it hits an unhelmeted head.
