Dear Sir,

Please contact your area inspector and the local office of ESIC to submit the new DF biometric form for the employee. The department will arrange a camp for the preparation of the smart card. This card is valid all over India in the panel of ESIC and all dispensaries. There are two separate cards, one for the family and one for the employees. In the future, it will be very useful for insured persons. I have heard that starting from August 2010, ESIC dispensaries only accept the smart card for medical benefits. It may be useful for you. After computerization of ESI Branch Offices effective 1.8.10, certain details such as the name of the employer, address of the I.P., etc., can be changed by the concerned Branch Manager online. Furthermore, I feel that your question is not clear.

Hi,

I could not go when ESI camp was conducted in our company for ESI smart card. Now, how can I apply for the same? I have my ESI number as well. Can I directly apply, or do I have to wait until our company conducts this camp again in the future?

Hi,

As I understand it, one card is for the employee and another is for his family members/dependents. For the family members' card, a group photograph has to be attached with the form, or all family members should go to the ESIC office to have their photo taken. Could you please clarify?

No, all your versions will be wrong. I will clarify the doubts one by one:

01. If a person fails to attend the Photo Camp: No need to worry, the camp will be conducted again in your area. Until then, you have to wait. There is nothing to worry about. Even though it was fully computerized until it was fully streamlined, manual cards will also be accepted for treatment. Additionally, as an initial stage, WIPRO employees are conducting the Photo Camp. In the later stages, the concerned Branch Office will handle this work exclusively.

02. In case of the addition of family members: Initially, the employee was provided with only a single photo. The cards provided initially are permanent; there is no question of adding other members by providing photos along with any form. However, as mentioned, in the later stages, this process will be handled at the Branch Office level. Whoever they want to add must be brought to the local office along with the smart card, and the concerned Branch manager will add the details of the additional family members to the backend data of the software.

Don't worry about the ESI Smart Card. Once you are covered under ESI and enrolled by submitting the Declaration Form to the concerned Branch Office through your employer, it is the duty of the ESIC to provide the smart card to you. Just inquire at the concerned Branch Office about the probable date for the photo session in your area for the smart card.

Dear Gusha,

Don't break your head regarding the Photo camp. Primarily, ESIC is taking it as its primary responsibility to provide the smart card to all the eligible insured persons. Regarding the details of the photo session, kindly enquire about the tentative dates with your ESI Local Office Manager.

Until the computerization gains pace, even manual cards will also be accepted. However, it is always advised to register under Smart Card at the earliest.

Dear Sir,

I want to know if a company is registered with the ESI Corporation in Delhi, and if an employee works in another state, can the employee still receive ESIC-related benefits in that state?
