I am attaching a simple format for petty cash expenses report. This is very easy to keep track and audit. At the month end just consolidate the summary and post it in the ledger. This report has to be supported by vouchers & bills, else it is a total collapse.

What do you want to show in your patty cash. There are many idea for patty Cash format but patty cash is used for only expenses are not for asset purchase and for very small amount.
