Dear XXXXXXXXXX,
This is just a suggestion. You can alter this as per your requirement:
This is to inform you that you have failed to perform your duties properly by not completing the task assigned to you and also misled the project lead by claiming that you had finished the set of tasks assigned to you.
Additionally, you were found using your personal email ID to chat with outsiders, which is against our organization's regulations.
Effective immediately, you are expected to be more responsible and comply with the rules and regulations of the organization. Further incidents of this nature will result in strict disciplinary actions against you.

Dear Thulasi,
The following is a format. Make suitable changes accordingly.
Dear Mr. XXXXXXXX,
It is observed that in the recent past, your performance has been very poor and not up to the mark. You are not reaching your targets, completing the work within the stipulated time and schedule, or adhering to the instructions of your superiors issued to you from time to time. It is also noticed that you are misusing work facilities like computers provided for official use during working hours. Your actions are causing disruption to the day-to-day work. You are aware that ours is a time-bound organization, and tasks should be completed as scheduled.
Your indifferent attitude towards your work and superiors amounts to misconduct and may lead to disciplinary proceedings.
However, you are hereby severely warned and advised, in your own interest, to be more attentive in your work, complete tasks as scheduled, follow instructions from your superiors, and improve your behavior.
In case of any repetition of the above actions in the future, the management will have no option but to initiate necessary disciplinary proceedings against you.
Hope this will serve the purpose.
In case of any doubt, feel free to contact.
Thank you.

The Warning Letter or MEMO - as usually known, is a first step towards initiating a reform or disciplinary action against a delinquent employee. Such communication should initially be polite and must convey a message strongly to him that he is being found lacking on certain counts and that he should pay special attention to those points. Remind him/her that the company has high expectations from him and wants him to grow with it.
If the issue persists, a second Memo should follow, expressing some concern and conveying displeasure while asking him to be more careful. Inform him that his actions will be monitored closely, and he will be under scrutiny.
The third communication could be a formal disciplinary one. Notify him that despite the opportunities given to him for improvement, he has not met expectations. If this behavior continues, the company will have no choice but to proceed with disciplinary actions.
Finally, issue a show cause notice as to why his services may be terminated as per the law. Request his explanation within a specified timeframe. Upon receiving (or not receiving) a response, take action according to the terms and conditions of his employment contract and the law, which could range from a warning to termination or dismissal.
In the meantime, supplement your actions with meetings, training sessions, problem-solving, and other managerial strategies to address the situation. Keep a record of all interventions.
