Good morning! There is no difference in formats under the ESI and PF Acts from state to state because these are central Acts. The only difference is the language used. The forms can be printed in the language known there.

Simply enter the name and other details of the new employee in this register and assign the PF account number sequentially. Complete Form-2, the nomination form for that employee, and attach it with Form-5 (return of new joiners). Send these documents to the RPFC office within 15 days of the subsequent month.
