I have a suggestion which you might want to look at. Rather than having just one progress report prior to confirmation, if your probation period is 6 months, it would be more constructive to have a mid term review at 3 months of a person joining by when if there are any major concerns regarding performance, it can be conveyed to the candidate and action plan drawn up to better the performance. This will avoid future hassles like extension of probation and other legalities involved.
Roma's form has captured all essence of a progress report. You can use the same format for the mid term review too.
