I need some good format for Performance Review & Apprasial Letter. This is the one we want to issue to employees on completion of their performance review. We have Performance Review Form in place and that is mailed to employees one filled up by supervisors. Now I want to introduce letter also. Can some please help in this?

As per the appraisal meeting and recognizing your effort and performance for the last year, we are pleased to inform you that your gross salary has been revised by ____ per annum, effective from 1st October.

Your salary details are strictly private and confidential. The details in this email must not be disclosed or discussed with others. Please acknowledge your acceptance of the revised terms.

We would like to take this opportunity to express our appreciation for your contribution during the past financial year. We look forward to your continued support in the future.

Hi Maulik,

In addition to Mitali's format where the outcome of an appraisal is positive, there are times when the opposite is true.

Here is a format for when that is the case:

Dear [Recipient's Name],

On DD/MM/YYYY, you attended a Performance and Appraisal review meeting. After carefully considering the discussions, I regret to inform you that your performance to date has been below expectations. The reasons for this are (insert reasons). We expect your performance to improve substantially now, and we will monitor your progress over the next 3 months. If there is no improvement, you may be subject to disciplinary action, which could lead to the termination of your employment. As a result of this decision, you will not be awarded an increase in your gross salary.

If you feel that you require further mentoring or training, please discuss this initially with your manager. This letter will remain in your personnel file and will be referenced at your next appraisal. Should you wish to discuss this decision, feel free to contact me; otherwise, please acknowledge this letter by signing and dating the enclosed copy.
