I find your first warning itself is so ominous and there is no second chance for the employee to explain. After all HR means Human Resource and not Human Destruction. Of course, I agree where there is perennial absence or frequent absence without notice, you take action. But not at the first instance....you then lose your credibility. Many HR managers act like Hitler...but why???

Im agree with wencat. this warning letter can be used for giving second or third warning but not for the first time.
I personaly believe and have experience that if u genuinely take interest to know the casue of absenteesm there is no need for such memo. but i know it is only possible in small organization. Im working in retail industry and that's able to heat from everybody in detail.
