I resigned from my job in September 2007. Currently, I am working with an organization that does not offer a PF facility. Therefore, I would like to close and withdraw the money from my PF account.
Please provide me with information on the proper form required to withdraw the money. If possible, kindly send the form format as an attachment to my email address (nkrajib@gmail.com).

To proceed, you need to fill Form No. 19 (Employees' Provident Fund Scheme, 1952) and Form No. 10C (Employees' Pension Scheme, 1995). You can then withdraw the PF deposited by your previous employer into your account.

It is not mandatory to get a signature from your previous employer; you can also take the signature of your Bank Branch Manager and submit it directly to the PF Department.

Are you sure about this? As per practice, we have to fill both PF and Pension forms and send them to our previous employer, who attaches copies of challans and calculations of the total PF amount, then submits them to the PF department for processing.
Please revert with the method to be followed in case the same can be withdrawn through the bank. I am also facing the same problem since I left my first employer in 2005, and I have been following up with them, but they are not responding. Your reply will clear the way for many of us.
