Kindly provide me with the format of PF Closure Form on an urgent basis. As far as I know Form 10c+19 is used for the same, but I do not have an idea of the format. Request you for the same.

buy PF withdraw form (form no.19 and 10C) from the local PF office of the cities where u worked.
U need three forms to be submitted to three organsiations where u worked. some part of the form is to be filled by you (forms are easy to fill) and rest to be filled by your previous companies. These companied will fill their part and submit the same to PF authorities. From there you will receive the amount directly in your mentioned account number with a information note to your mentioed home address.
