Notice of Inquiry
It's called a Notice of Inquiry, which should contain the following:
a) The need for an inquiry as not satisfied with the explanation in response to the Show-cause letter.
b) Date, time, and place of the inquiry.
c) The charge or charges, which should be specific.
d) Request the complainant to bring witnesses, if any.
e) A sentence to state that if the complainant fails to attend, the inquiry will proceed ex parte.
