Epf calculation - What is the actual method as per EPF Act which states 12% of Salary/Wage?

One of the contractor has submitted Labour Declaration to a PSU with EPF deductions calculated only 12% on Basic excluding DA is there any amendments in EPF. Some contractors submit declaration on the basis of 12% of Basic+DA. What is the actual method as per EPF Act which states 12% of Salary/Wage
