At the time of relieving, I forgot to ask my HR to credit my earned leaves/privilege leaves, which the company policy also states. It's been more than a year now as I was on a break due to maternity. Am I eligible to ask for the credit of my earned leaves/privilege leaves?

If the company policy provides it, you are certainly eligible to get the encashment of PL/EL. The HR department should have initiated the same even without your asking. Please get in touch with your past employer by providing the necessary details. What about other eligibility like PF, EPS, etc.
