For this type of activity, you need to have a proper legal agreement. You need to make a list of the services that the service provider needs to provide, where he will provide them, how he will provide them, when he will provide them, how his quality of work will be measured, how his invoices will be processed, and what the payment terms will be. In addition to this, you need to anticipate the communication requirements between both of you and include as an appendix the format of the various reports and returns.

Your goal should be to make the agreement as foolproof as possible. This will help you avoid disputes in the future.

Instead of calling it a letter of authorization, in common parlance, it is referred to as a work order or agreement. Some hiring consultants may have a format of enrollment available, so you may want to check with them. Alternatively, you can create one on your own with all the necessary terms.

It should essentially be an agreement format with all terms and conditions clearly mentioned, including the validity period, and should be duly executable as advised by our seniors here. The contents may vary depending on the type of industry you are in (manufacturing, software, BPO, etc).
