I need help as I want to create a letter that states if an employee leaves within a year of receiving training that the company has funded, they will be required to reimburse the cost of the training. Is there a specific format for this type of letter?

Ideally this should have been got signed before sending him on training. What happens if he refuses?

Dear colleague,

Don't you observe that in this forum, the very large majority are posting with their names, but you are choosing to hide yours. Is there any valid reason?

Secondly, you should have attempted a draft of the letter yourself first, uploaded it, and requested for value addition instead of spoon-feeding.
