I just want to know that is this possible , if an employee does not want to deducted his PF and ESIC. Is there is any type of Form there through which employee PF and ESIC is not deducted.

As per statutory compliance rule , there is no rule of this . Its a compulsory .
A company or establishment is liable for remitting PF both on behalf of itself and its employees if it fulfills the requirement of coverage under the EPF & MP Act 1952 :
more than 4 employees in case of Cinema theatres
more than 49 employees in case of Co-operative Societies working without the aid of power.
more than 19 employees in factories and other establishments notified by the Central Govern...
