However, in general, you can have a systematic approach by taking the service of a qualified safety personnel. In a broad sense, look for unsafe acts and unsafe practices. Once the above two are known, you can consider implementing the corrective action/mitigation plan after taking the help of the functional team. The monitoring, as above, is a continuous process.
