Dear Sajid, You won’t learn much if someone gives you a ready-made questionnaire. Hence, please access the web on the topic in question and see what others have done, how and why that way.

I am not fully conversant with where you would be conducting the survey, whether the workers/staff there understand English and have access to the internet, etc. Also, you have not given me the aim and objectives of your study and what sort of sampling you will do, etc.
Hence, I suggest that you access and study some academic papers written on the topic or other related topics and tell me what you intend to do, how and why? In trying to answer my questions, you will find the answer to your question.
